2010 Plan and Payment Information
We are writing to tell you about your rates for the coming year.
Information on your plan(s), rate(s),and monthly payments are printed
on the back of this letter. The amount will be deducted automatically
each month from your bank account by Electronic Funds Transfer (EFT).
If there has been any change in your banking information. please tell
us right away so you won't miss any payments.

At UnitedHealthcare Insurance Company, we take pride in keeping the
cost of your coverage as low as possible. Like many health insurers,
we had to increase rates for some plans to keep up with the rising
health care costs.

If you are thinking of going bankrupt because of health care bills,
